Affiliates
In order to become a Supportive Systems, LLC Affiliate you must:
- Have a minimum of a Master's degree in a mental health discipline and three years experience.
- Submit proof of current licensure/certification in your state
- Submit proof of professional liability insurance (minimum required limits are $1,000,000 per claim/ $3,000,000 aggregate.)
- Submit your current resume
- Submit a completed Supportive Systems, LLC Individual Application
- Submit Affiliate & Business Associate Agreements
